Transportation Services

Need assistance getting to doctors’ appointments or running errands? PALS transportation service is offered to residents of San Juan County, but you must first have a quick in-home assessment conducted by a PALS staff member. Then, you will be signed up for our services. Please call one week in advance to schedule a pick-up time. There is a 2-hour minimum required for each transportation reservation.

Can you complete the following, “I’ve fallen and I can’t …?” It is vital to your health and safety to get the help you need quickly when you have fallen in your home. That is why PALS has partnered with Philips Lifeline Medical Alert Services. We offer an array of products through Philips Lifeline that will provide the assistance you need if you fall. Call Partners Assistance Living Services today and we will install the Medical Alert System that works best for you tomorrow.

Philips Medication dispensers

Avoid a trip to the emergency room because you forgot to take your pills. Partners Assisted Living Services can install and maintain Philips Medication Dispensers that assist you with taking your medication on schedule, from convenient pre-filled dosage cups. It’s easy and prevents incorrect medication use.

What is Virtual Care? It is part of PALS SafeKey WellCare menu of services, and it can help PALS’ clients avoid hospital admissions or re-admissions or assist clients who are not able to comply with diet, medication or self-monitoring.

HomMed monitoring allows remote in-home monitoring of the clients blood-pressure, pulse, oxygen saturation, heart rate, and weight by a health care professional. It’s like having a nurse in the living room. LiveWell! HomeCare.

Safety Assessment

It’s important to be safe in your home, which is why Partners Assisted Living Services offers in-home safety assessments. We will check your home for any trip hazards, make sure there are clear fire-escape routes, check batteries in fire alarm systems, etc., to ensure you are safe and comfortable in your home.

Free In-home Assessment

The best way for us to identify your needs is completing a free in-home assessment. An assessment helps PALS explore areas of concern that may be overlooked. We will meet with you and family in your home and discuss your lifestyle and special concerns.

Based upon the information gathered at the assessment, PALS will develop an individualized plan of care that meets your own unique set of needs.
